REQUIRED NFCYM/NCYC RELEASE FORMS
NFCYM requires that every youth and adult participant complete the appropriate NFCYM/NCYC release form. These are two page forms with separate forms for adults and youth. All participants may download them directly from this website below. The forms DO NOT need to be notarized as it is NOT required by our diocese. These forms should be completed between now and September 6, 2019.
One completed copy of the form per person should be submitted to Diocese of Knoxville Youth Ministry Office with registration payment. You should retain two copies: one for your records and one to return to the participant on-site and which should be kept in his/her name badge holder.
These forms should be completed between July 1, 2019, and September 6, 2019.
The NFCYM/NCYC release forms are now available in English.

Spanish translations of the forms should be available by the end of next week. If you are not already aware, EACH member of your delegation must complete the appropriate form, which should be submitted with a copy of your registration invoice and fees. You should keep a copy of each form to return to participants onsite and which should be kept in the participants’ name badge holder during NCYC.

NOTE: The release form is NOT a registration form. It does not include all of the fields that are needed to complete registration. Also, you will need to provide youth participants a separate medical permission form as the NCYC release form does not address allergies, medical conditions, or medications.

NOTE: The waiver forms should not be used to collect registration data. The waiver does not ask all the information required for online registration. Delegation or group leaders should design a separate collection method.
REFERENCED CODES OF CONDUCT
Below are links to the codes of conduct referenced in the adult and youth release forms. The codes of conduct do not need to be signed as they are incorporated into the release forms by reference. If you wish to provide a hard copy of the text, the pages are in PDF format for easy printing.
MEDICAL RELEASE FORM
In addition to the NFCYM/NCYC release form, The Diocese of Knoxville requires that each attendee (adults included) complete a medical release form that includes information about allergies, medications being taken, authorization to provide over-the-counter (OTC) medication, and medical insurance information, among other things.
The Diocese of Knoxville advises all Parish Coordinators to collect and retain a copy and to return a copy to the participant on-site to be kept in his/her name badge holder in the event of an emergency.

There are two ways to input your initial data: through an Excel template or one-by-one. You can use either method exclusively or do a combination of both methods. The choice is yours. The beauty of using the Excel template is that you can copy duplicate information for multiple records with fewer keystrokes.

Note: Beginning October 1, 2019, all editing capabilities will be locked. Corrections will have to be made in consultation with Progress In Planning (PIP). Substitutions will incur a $20 per person fee. Cancellation requests must be submitted in writing no later than November 1. Cancellation requests received after November 1 will be considered on a case-by-case basis. Refunds for cancelled registrations will be processed in January 2020 minus a $40 cancellation fee per person.

REGISTERING VIA EXCEL METHOD: To import registrations, you must use the 2019 spreadsheet template (below). No other spreadsheet formats are supported. There are drop down options assigned to specific columns (fields). An initial selection must be made from the menu, which can then be pasted into subsequent cells.

Safety and Security at NCYC
In light of recent events, you should have received an email this morning with information about security measures at NCYC. If you did not receive this email, a PDF copy of the text is available here.

As you begin to prepare your delegates for the pilgrimage to NCYC, please encourage them to bring a Bible. We will incorporating links to an online Bible into the 2019 NCYC app, but it’s not the same as holding a hard copy of Scripture in one’s hand.Invite Your Youth to Lead the Way at NCYC – Youth Ambassador Registration Now Open! 
The Youth Ambassador program allows for more participation in NCYC by young people from across the country. Each delegation is encouraged to send multiple teams from their diocese or delegation (a team consists of four youth and one adult liaison) . These youth leaders will provide a welcoming presence and prayerful introductions and assistance at workshops and mega sessions at NCYC. There is no limit to the number of teams a delegation may submit. Highlight Your Youth’s Talents at NCYC’s Top Talent 
NCYC’S Top Talent is a talent contest held during the 2019 NCYC. Contestants must be high school students in attendance at the 2019 NCYC in Indianapolis. Audition videos/demo recordings will be accepted until October 31. Contestants will be chosen to audition in person at NCYC on Friday, November 22, with the top ten finalists performing on Saturday, November 23, in front of a live audience and a panel of expert judges who will select a winner. Share the promo video and Top Talent website with your NCYC attendees so they can begin to prepare for their live auditions. Visit www.ncycstoptalent.org.

SEARCH for Christian Maturity is a retreat program that tries to help high school juniors and seniors see how they can be faith leaders in their schools and parishes. This is done by having the participants come to know God, themselves, and others in a more loving, personal way. SEARCH is not a means purposely set up to correct one’s character, morals, emotions or troubled school or family life. It is a method of providing people with a way of encountering themselves, others and God in a way which is unique and powerful. SEARCH helps people to see the need to bring their spiritual life into more conscious contact with their day-to-day living. The essential component of SEARCH is its youth-to-youth approach, referred to as peer ministry. This component cannot be stressed enough; other programs can be valuable, but no one can talk with and relate to another better than a peer. So, SEARCH is basically a weekend experience in Christian living. It is designed to help people see their value as human beings and members of a loving community. This happens as the team and the Searchers share their thoughts, feelings and ideas. Participants are then challenged to take their growth and ideas back to their families, schools and parish communities. Central Elements There are six elements that we hope each SEARCH weekend will highlight and that each participant will take away from the retreat. These central elements are: Trust               Sharing               Community               Prayer               Growth               Fun SEARCH aim is to help Catholic high school juniors and seniors take a fresh look at their own faith journey, reflecting on their place within their families, schools, parishes and community as a disciple of Christ. This is accomplished through activities and experiences that lead participants to a renewed understanding of Christ at work in their lives. SEARCH is facilitated by teens for teens with the guidance of adult SEARCH Coordinators. The main focus of the retreat is three fold: Motivating the youth to seek a personal relationship with Christ. Getting young people involved with their respective parishes. Picture of Love Marriage Preparation Program for Engaged Couples

This marriage preparation program is a supplement to the couple’s marriage formation with their parish priest or deacon. The Picture of Love marriage preparation program for engaged couples explores the joys and challenges of living out the Sacrament of Matrimony with special focus on the importance of inviting Jesus to be the center of marriage and family life. The program will help the couple gain insights into their relationship, as well as give them practical ideas and tools to help smooth their journey and become the “Picture of Love” to one another.

Offered 3 times a year • Cost is $135.00 per couple (Program certificate will get the couple a $60.00 discount on their marriage license.) You must attend the entire program to receive your certificate.

The program is 7:00 – 10:00 p.m. on Friday and 9:00 a.m. – 6:00 p.m on Saturday. Snacks, a light breakfast, lunch and Saturday dinner are included.

This program is sponsored by the Diocese of Knoxville Office of Marriage Preparation and Enrichment in conjunction with the Chattanooga Deanery.
