Information on the application process and eligibility requirements for retirement benefits from the 403b and Lay Retirement Plan are contained in the following documents:

There are three forms necessary to begin the process of receiving retirement benefits. The forms may be downloaded below. Please return them to the staff person at your place of employment who manages employee benefits.

Important note: retired employees are not eligible to remain on the diocesan health insurance plan.

Important information for Medicare beneficiaries:

Each year Medicare-eligible employees receive a Medicare Part D notice from Caremark. It is essential that you retain this notice as you will need it when you are no longer eligible for the diocesan health plan.
